You may use this form to report any concern, risk, or observed negative outcome related to Lynes’ AI features.
Examples include:
- Incorrect or misleading AI outputs, such as inaccurate transcriptions or unintended chatbot responses.
- Privacy or data protection issues, such as personal information being processed or shared unexpectedly.
- Ethical or fairness concerns, such as potential bias, discrimination, or unfair treatment.
- Safety or security risks, such as system misuse, unexpected AI behaviour, or vulnerabilities.
- Other impacts, such as social, reputational, or environmental harm linked to AI use.
If you are unsure whether your observation qualifies — please report it anyway. Our team will assess all submissions with care.
How we handle your report
- Every report is reviewed by our AI Governance Team and assigned a severity level.
- Serious or high-risk cases are escalated to Lynes’ AIMS Management Leader for immediate investigation.
- We may contact you for additional details if you have provided your contact information.
- Reports are handled confidentially.
